Discussed required online incident reporting. Discussed the upcoming implementation of fingerprinting for all staff.
An unannounced monitoring inspection was conducted this date. A total of 89 children with 15 staff present. Children were observed playing outside on the playground and transitioning back inside. Observation of Face to Name was made, children aware of the procedures of lining up to return to the classrooms. Diapering and handwashing procedures observed. Staff interviews conducted to determine proficiency with emergency procedures. Two parents interviewed while on site. Children were engaged in circle time, table top activities which included arranging shapes and colors in patterns. Staff strategically placed on the playground to ensure proper safety and supervision. A total of 10 children's files and 8 staff files were reviewed. All medications reviewed. Standard #: 22VAC40-185-130-A Description: Based on a review of 10 children's records, 1 child record did not have immunization documentation. Evidence: Child #2 began care on 7/5/17. No documentation of immunizations was found for this child. Plan of Correction: We have been requesting this documentation from the parents. We will obtain the documentation by this week.
Standard #: 22VAC40-185-140-A Description: Based on a review of 10 children's records, 1 child record did not have documentation of a physical exam. Evidence: Child #2 began care on 7/5/17. No documentation that this child has had a physical exam within 30 days of the child's enrollment date. Plan of Correction: We have been requesting a health exam from the parents. We will ensure a health exam is obtained this week.
Standard #: 22VAC40-191-40-D-3-D Description: Based on review of 8 staff records, 3 staff records were found incomplete. Evidence: Staff #1, #2 and #3 have incomplete Sworn Statements completed in November 2017. The Sworn Statements reviewed do not include information about residency outside of Virginia. Staff #2 does not have a Central Registry Search on file. Plan of Correction: We will relay this information to our Corporate office since these staff do not work directly at this center.
Standard #: 63.2-1721.1-B-3 Description: Based on review of 8 staff records, 3 out of the 8 staff records do not have a child abuse and neglect registry search from the state these employees reside in. Evidence: Staff #1,#2 and #3 have lived outside of Virginia in the past five years. Copies of results of a search from a central registry from a state other than Virginia was not obtained. Plan of Correction: Contact will be made with the corporate office, these three staff do not work directly at this center.
