An unannounced Monitoring Inspection was conducted on November 28, 2017 from approximately 3:45 p.m. until 6:05 p.m. There were 14 children and 1 staff and 2 volunteers present with children . The record review sample consisted of 5 children's records and 2 staff records. Children were observed eating snack, working on homework and playing games. Standard #: 22VAC40-185-60-A Description: Based on a record review and staff interviews, the licensee did not ensure to maintain and keep at the center a separate record for each child enrolled which contains all the required information. Evidence: During the inspection on November 28, 2017, 2 out of 5 records reviewed were determined to be incomplete. 1) The record for child #1 did not contain the addresses for both emergency contacts listed 2) The record for child #2 did not include an address for the second emergency contact listed. Plan of Correction: 2nd parent info completed by parents.
Standard #: 22VAC40-185-280-B Description: Based on a record review and staff interviews, the licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection on November 28, 2017, in the office on a desk were 2 spray bottles of disinfectant, an air fresher and a container of "Goof off". All items were labeled "keep out of reach of children" and at least one of the other statements "warning", "hazardous" and "flammable". Plan of Correction: Office doors will be closed and locked.
Standard #: 22VAC40-185-550-D Description: Based on documentation review and staff interviews, the licensee did not ensure the center implemented a monthly practice evacuation drill and a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios. Evidence: During the inspection on November 28,2017, there was not documentation for monthly evacuation drills for June, 2017, July 2017and September 2017. Plan of Correction: Risk management team will add program center to calendar for monthly practice drills.
Standard #: 22VAC40-191-60-C-1 Description: Based on a record review and staff interviews, the licensee did not ensure all employees had an original criminal history record report (CRC) within 30 days of employment or volunteer service; or employment shall be denied. Evidence: During the inspection on November 28, 2017, staff #2 did not have a CRC. Staff #2's date of hire was 8-9-2017 and was confirmed by their supervisor. Staff #2 was observed working in the center during the inspection. Plan of Correction: Staff #2 and 3 other staff members have filed for proper CRC. Applications mailed 11/29/2017.
