Licensing inspector discussed new sworn disclosure forms and new forms to reflect the name and phone number of emergency contact with the administrator.
An unannounced monitoring inspection was conducted today from approximately 12:20pm-2:50pm. There were 62 children in care directly supervised by 12 staff. A sample of 6 children?s records and 4 staff records were reviewed. The children were observed playing with toys, reading, napping and eating. 1new staff has been hired since the last inspection. Standard #: 22VAC40-185-60-A Description: Based on record review, children's records were incomplete. Evidence: 6 of 6 records reviewed were incomplete. 1) Child A was missing the employer phone number of both parents. 2) Child B was missing the addresses of 2 of 2 emergency contacts. 3) Child C was missing an annual update. File was last updated 8/9/16. 4) Child D was missing an annual update. File was last updated 9/7/16. 5) Child E was missing annual update. File was last updated 9/3/15/ 6) Child F was missing an annual update. File was last update 6/2/16. Plan of Correction: I will update files tomorrow.
Standard #: 22VAC40-185-70-A Description: Based on records reviewed, staff records were incomplete. Evidence: 4 of 4 staff records were incomplete. 1) Staff A hired 10/2/17 was missing orientation. 2) Staff B, C and D were missing emergency contacts. 3) Staff D was missing 1 reference. Plan of Correction: I will have staff update the files.
Standard #: 22VAC40-185-280-B Description: Based on observation, cleaning materials were not kept in a locked place. Evidence: All purpose cleaning wipes were found in an unlocked cabinet in the "Montessori 4" room. Plan of Correction: They will lock cleaning supplies now.
Standard #: 22VAC40-185-290-3 Description: Based on observation, outlets were not protected with protective covers. Evidence: 1) 1 outlet wasn't protected with a protective cover near the sink in the "Toddler's 2" room. 2) 2 outlets were uncovered on a power strip near the phone in the room where the Montessori 3 room and the Montessori 2 room take naps. 3) 1 outlet near the cubbies in the Montessori 3 room was uncovered. 4) 3 outlets near the cubbies and 2 outlets near the bathroom were uncovered in the Montessori 2 room. Plan of Correction: They will be covered.
Standard #: 22VAC40-185-550-M Description: Based on injury records reviewed, injury records were incomplete. Evidence: Of 5 injury records reviewed the following was missing: 4 were missing the parent notification, 4 were missing future action, 1 was missing treatment given, 2 were missing time of incident, and 1 was missing way in which parent was notified. Plan of Correction: I will talk to staff today.
Standard #: 22VAC40-185-560-G Description: Based on observation, food brought from home was not labeled. Evidence: 1) 3 lunch boxes in the Montessori 1 room were not labeled with the name and date of the child. 2) 1 lunch box in the Montessori 4 room was not labeled with the name and date of the child. 3) 4 lunch boxes in the Toddler 1 room were not labeled with the name and date of the child. Plan of Correction: Tell parents and teachers to put dates on lunches brought from home.
