Technical assistance was provided in the following areas of the standards: 22VAC40-185-(7)-550-D (Shelter-in-place drills); 22VAC40-191 (Background checks)
An unannounced monitoring inspection was conducted on 12/6/17 from 9:55am - 12:00pm. During the inspection there were 54 children ages three months old to two years old in care with 16 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, playing outside, eating lunch and during rest period. Records were reviewed for five children and ten staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.
Standard #: 22VAC40-185-130-B Description: Based on a review of five children's records, it was determined that the facility did not ensure that they obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #4 (date of birth 7/19/16) contained an immunization record that was dated 10/7/16. 2. Staff #11 (Program Director) reviewed the record for child #4, and confirmed that an updated immunization had not been received. Plan of Correction: The facility responded: The parents of child #4 have been asked to provide an updated immunization record prior to today's inspection. We will make another request to receive the updated immunization record. Center management will review the records of all children under the age of two years old to ensure they have an updated immunization record.
Standard #: 22VAC40-191-60-C-1 Description: Based on a review of ten staff records the facility did not deny continued employment of a staff who did not have an original criminal history record report within 30 days of employment. Evidence: 1. Staff #1 (date of hire 10/2/17) had no documentation of a completed criminal history record report. There was a copy of the criminal history report that indicated it was mailed, but no other follow up was completed prior to the inspection. 2. Staff #5 (date of hire 10/16/17) had no documentation of a completed criminal history record report. There was a copy of the criminal history report that indicated it was mailed, but no other follow up was completed prior to the inspection. 3.Staff #10 (date of hire 8/6/17) had no documentation of a completed criminal history record report. There was a copy of the criminal history report that indicated it was mailed, but no other follow up was completed prior to the inspection. 4. Staff #11 reviewed the records for staff #1, staff #5, and staff #10, and confirmed that a completed criminal history record report had not been received within 30 days. Plan of Correction: The facility responded: The Virginia State Police was contacted, and stated that staff #5 and staff #10 had not been processed yet, but that staff #10 was completed on 8/31/17, so a new copy of it will be mailed to the facility. If any new employee's criminal record check is not received within 30 days of employment, we will contact the Virginia State Police and document it.
