An unannounced renewal inspection was conducted on 12/01/2017 from 10:30am - 2:00pm. At the time of entrance there were 25 children, ages infant - four years, in care with five staff. Children were observed during morning program time which included outdoor play. Lunch and nap was additionally observed. Records were reviewed for six staff and eight children. Medication, emergency supplies, transportation procedures, hand washing, sanitizing of tables, diapering and written emergency plans were also reviewed and observed during the inspection. The violations were reviewed with the program director at the conclusion of the inspection and are listed on the violation notice.
Standard #: 22VAC40-185-260-A Description: Based on record review and interview, the center failed to obtain an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: 1. The most recent fire inspection on file was conducted on 05/09/2016. 2. The local fire official's office confirmed that a more recent fire inspection had not been conducted. Plan of Correction: An updated fire inspection has been requested.
Standard #: 22VAC40-185-420-E-1 Description: Based on record review, the center failed to ensure that for each infant the daily record shall contain a description and time of bowel movements. Evidence: 1. The infant daily activity sheet for child 1 lacked any written documentation of any diaper changes after 10:20am on 11/29/2017. Child 1 was in care until 2:31pm on 11/29/2017. a. No diaper changes were noted after 8:00am on 11/30/2017. Child 1 was in care until 3:55pm on 11/30/2017. 2. The infant daily activity sheet for child 2 lacked any written documentation of any diaper changes after 11:00am on 11/30/2017. Child 2 was in care until 3:55pm on 11/30/2017. 3. The infant daily activity sheet for child 3 lacked any written documentation of any diaper changes after 10:00am on 11/30/2017. Child 3 was in care until 5:12pm on 11/30/2017. Plan of Correction: The director stated: The diapers for each child was changed and/or checked every hour however, the actual change was not documented on the activity sheet. All staff will be re-trained on proper and accurate completion of infant sheets each day to include documentation of each diaper change.
Standard #: 22VAC40-80-120-E-7 Description: Based on observation, the center failed to ensure that a copy of the notice of intent, issued by the department on June 9, 2017 was posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations. Evidence: The notice of intent was not posted as required by the Special Order issued on July 7, 2017. Plan of Correction: The notice of intent was posted during the inspection.
