22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 11:35am-2:45pm. There were 82 children (2-5yrs.) directly supervised by 15 staff members. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, building with blocks, listening to stories and participating in circle time. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Standard #: 22VAC40-191-60-B Description: Based on staff record review, a new staff member did not have the updated Sworn Statement or Affirmation form required for all new employees hired after July 1st, 2017. Evidence: Staff#1 did not have the updated Sworn Statement or Affirmation form. The Sworn Statement or Affirmation form on file is outdated. Staff began working at the center on 8/21/17. Plan of Correction: We will print and have the form completed today.
