Technical Assistance:
Please ensure that any new staff hired after July 4, 2017 have the new Sworn Disclosure of Affirmation form completed and that the appropriate out of state Search of the Central Registry for the appropriate agency is requested for any staff indicating residence outside of Virginia in the last 5 years.

Comments:
An unannounced monitoring inspection was conducted from1:15 p.m. to 4:50 p.m. At the time of entrance the following adult to child ratios were observed: Room 1: Six infants, 2 to 9 months old, with 2 staff Room 2: Eight infants, 9 to 13 months old, with 2 staff Room 3: 16 toddlers, 14 to 23 months old with 2 staff (naptime) Room 4: 14 children, 4 years old, with 2 staff Room 5: 21 children, 3 years old, with 2 staff Room 6: 24 children, 2 years old, with 3 staff The records sample size consisted of 9 children?s records and 8 staff records. Children were observed resting quietly at nap time, singing songs, dancing to music, playing with toys and having an afternoon snack. Infants were observed have floor play and tummy time, being held, bottle fed, rocked, cuddled and sung to. Restroom procedures, diapering procedures, hand washing, food service and sanitizing were also observed. Medication administration was not observed. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance.

Standard #: 63.2-1720.1-B-3
Description: (NEW REQUIREMENT) Based on a review of records and interview, the licensee failed to request a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him. Evidence: Eight staff records were reviewed during today's inspection. The record for staff #6 indicated via the Sworn Disclosure or Affirmation statement that staff #6 resided in a state other than Virginia within the last 5 years. However, the licensee had not requested a Search of the Central Registry from the State where staff #6 indicated she had resided within the past 5 years (Texas). When asked if a Search of the Central Registry for Texas had been requested, staff #1 stated that she had not been clear on the new requirement to obtain these background checks out of state and had not done so.

Plan of Correction: Staff #1 accessed the appropriate agency on line during the inspection and made a request for the Search of the Central Registry for staff #6. The Texas state agency replied stating that they do not fulfill these requests for other private agencies. Staff #1 printed and will maintain a copy of the request and response if staff #6's record showing that she has complied with the requirement to the best of her ability.
