Comments:
An unannounced complaint investigation was conducted to investigate a complaint related forbidden actions in the two year old classroom at the center. The investigation was conducted with the director beginning at 1:00 PM and ending at 2:30 PM. Four staff interviews were conducted. Two children?s record and three staff records were reviewed and in compliance. It was determined that the complaint was valid and a corrective action plan was developed by the center. Description: Based on staff interviews, it was determined that Staff A used the forbidden action of withholding food in the two year old room. Evidence: Staff stated that they observed staff A taking food away from and withholding food from children as a form of punishment for not using their utensils properly or playing with their food. The food was re-offered to the children upon redirection from staff A.

Plan of Correction: Written warning and specific training on positive discipline was immediately implemented, probation, reduction of hours, reassignment out of the twos class, reduction of duties.

Description: Based on staff interviews, it was determined that Staff A used the forbidden action of demeaning verbal remarks. Evidence: Staff stated that they observed staff A make demeaning remarks to children in the two year old classroom. During one observation, staff A referred to a child as a "clown" and stated that "clowns are not allowed in this class". In another observation, staff A was witnessed telling a child "you are not special".

Plan of Correction: Written warning and specific training on positive discipline was immediately implemented, probation, reduction of hours, reassignment out of the twos class, reduction of duties.
