A Subsidy Health and Safety Inspection was conducted on December 13, 2017 to review supplemental health and safety requirements. Licensing inspector arrived at 8:45am and departed at 4:30pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 18 children ages infant through five years old in care with three staff. Children were observed coloring, participating in center and circle time, and playing freely. The sample size was five children's records and five staff records. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-006-B-3 Description: Based on record review, in five of five children's records reviewed, the vendor did not ensure that there was a name, address and phone number for each custodial parent's place of employment. Evidence: There was no documentation of one or both parents' work addresses for all five children's records reviewed. Plan of Correction: Program Director will update all records with the parents' work addresses.
Standard #: VENDSUB-000-012-E Description: Based on observation and inspection of the facility, the vendor did not ensure that all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval: Current certification in CPR which includes an in-person competency demonstration; and Current certification in first aid. Evidence: The following staff members did not have documentation of CPR and First Aid Training within 30 days of employment or 90 days from subsidy vendor approval: Staff #1-hire date 10/02/17 Staff #3-hire date 9/19/17 Staff #5-hire date 10/17/07 Plan of Correction: Program Director will make sure staff have proper First Aid and CPR training and that the documentation is brought in.
Standard #: VENDSUB-000-031-A Description: Based on observation and inspection of the facility, the vendor did not ensure that the written emergency preparedness plan that addressed all required procedural components regarding staff responsibility and facility readiness with respect to emergency evacuation, relocation, lockdown and shelter-in-place procedures. Evidence: The center's written emergency preparedness plan did not include the following procedural components: Evacuation procedures did not include accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during evacuation or relocation. Plan of Correction: Program Director will make sure that the plan is easily accessible in the Director's Book.
