Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-185-(4)-330-B (Resilient surfacing); 22VAC40-185-(7)-500-A (Hand washing); 22VAC40-191 (Background checks)

Comments:
An unannounced monitoring inspection was conducted on 12/13/17 from 9:15am - 11:15am. During the inspection there were 70 children ages three months old to four years old in care with 21 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, and playing outside. Records were reviewed for eight children and seven staff. There was no medication being stored at the facility currently. Emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of seven staff records, it was determined that the facility did not ensure that staff resubmit updated TB results every 2 years. Evidence: 1. The record for staff #7 had documentation of a TB screening that was dated 8/24/15. 2. Staff #8 (Program Director) confirmed that an updated TB screening had not been received for staff #7.

Plan of Correction: The facility responded: Staff #7 will complete a new TB screening. All current staff will complete an updated TB screening every two from the previous TB screening.

Standard #: 22VAC40-191-60-C-1
Description: Based on a review of seven staff records the facility did not deny continued employment of a staff who did not have an original criminal history record report within 30 days of employment. Evidence: 1. The record or staff #4 (date of hire 6/12/17) contained documentation of a completed criminal history record report, but it was not the correct criminal record report. 2. Staff #8 (Program Director) reviewed the record for staff #4, and confirmed that staff #4 had completed the wrong criminal history record report.

Plan of Correction: The facility responded: Staff #4 will complete the correct criminal record check and it will be submitted to the Virginia State Police. If any new employee's criminal record check is not received within 30 days of employment, we will contact the Virginia State Police and document it.
