Discussed new requirements for backgound checks.
An unannounced monitoring inspection was conducted today from 11:30am-1:30pm. There were 39 children directly supervised by 8 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in small group play in the activity rooms, playing outside, and preparing for nap. The center was decorated for winter and learning centers were set up to reflect the theme. There were no medications on site today, which is consistent with center policy. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-150-B Description: Based on record review, not all children had an immunization report signed by a physician, his designee, or an official of a local health department. Evidence: Child 2's report of immunization is not signed by a physician, his designee, or an official of a local health department. Plan of Correction: I will ask the parent to get a signature.
Standard #: 22VAC40-185-160-C Description: Based on record review, not all staff have submitted at least every two years a negative Tuberculosis screening. Evidence: Staff 3's last tuberculosis screening was conducted on 8/6/2014. Plan of Correction: We will ask the staff to provide a new TB test.
Standard #: 22VAC40-185-270-A Description: Based on observation, not all areas of the center were maintained in a safe and operable condition. Evidence: 1. In the main classroom a carpet was observed to have warped causing the corner to curl upwards creating a tripping hazard. 2. Paint was observed to be chipped on the wall to the right of the main entrance. 3. The laminated corner of one of the tables in the main room has chipped off exposing the particle board beneath. 4. A carpet in the art room was observed to have warped causing the corner to curl upwards creating a tripping hazard. 5. On the playground a metal bracket securing a hose to plywood has a segment of overhanging sharp metal. Plan of Correction: We will tape down the carpets immediately. I will cut the excess metal today. We will replace the table top and repaint the chipped area of the wall.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children. Evidence: 1. Disinfectant wipes, sanitizing spray, and staff purses were observed in cabinets in the main classroom. One cabinet was unlocked and the other two were locked with the key stored in the keyhole. 2. Spray cleaner and aerosol air freshener were observed in the manager's office on an open shelf. 3. A can of aerosol air freshener was observed in the boy's rest room. 4. Hydrogen peroxide, borax, sanitizing wipes were observed in an unlocked cabinet in the art room. 5. A staff purse was observed on an open shelf in the library. Plan of Correction: We will lock all hazardous materials and keep the keep out of the cabinets.
Standard #: 22VAC40-191-60-C-1 Description: Based on record review, not all staff had a criminal record check on file within 30 days of employment. Evidence: Staff 5 does not have a returned criminal history check on file. Plan of Correction: We will follow up with the agency.
