- Technical Assistance:
New background check regulations and sworn statements, which can be found on the state website, were discussed with the center director during the inspection.
An unannounced monitoring inspection was completed from 10:20am until 1:30pm. At the time of the tour, there were 55 children in care with 10 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed playing outside, participating in centers and story time, eating lunch and hand washing. Medication, transportation, first aid and emergency supplies, children's injury reports, the emergency preparedness plan and other center documents were reviewed. Violations in the area of administration are identified on the violation notice and were discussed with the center director during the exit interview.
Standard #: 22VAC40-185-130-A Description: Based on record review and interview, the center did not obtain documentation of immunizations before a child can attend the center. Evidence: 1. The record for child #1 did not contain documentation of immunizations. Child #1 has a date of birth of 11/26/16 and a documented enrollment date of 9/27/17. Child #1 was in care on the day of the inspection. a. During the inspection, child #1's parent provided documentation of immunizations dated 12/13/17. 2. Staff #1 confirmed that there was no immunization record in child #1's record. Plan of Correction: The parent provided an immunization record for the child during the inspection, which was placed in the child's record. The parent stated the previous director did not request immunization records upon enrollment. Going forward, the center will ensure that immunization records for each child are obtained before the child's first day of care.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center did not obtain documentation of a physical exam by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. The record for child #1 did not contain documentation of a physical exam. Child #1 has a date of birth of 11/26/16 and a documented date of enrollment of 9/27/17. Child #1 was in care on the day of the inspection. a. During the inspection, child #1's parent provided documentation of a physical exam dated 12/13/17. 2. Staff #1 confirmed that the record for child #1 did not contain documentation of a physical exam. Plan of Correction: The parent provided documentation of a physical for the child during the inspection, which was placed in the child's record. The parent stated the previous director did not request documentation of a physical upon enrollment. Going forward, the center will ensure that physicals are obtained within the required time limit.
Standard #: 22VAC40-185-160-C Description: Based on record review and interview, the center did not ensure that at least every two years from the date of the first initial screening or testing that staff shall obtain and submit the results of a follow-up tuberculosis screening. Evidence: 1. The most recent documentation of a tuberculosis screening for staff #1 is dated 9/10/15. Staff #1 was working on the day of the inspection. 2. Staff #1 confirmed that her most recent tuberculosis screening found in the record is expired. Plan of Correction: Staff #1 stated she obtained a new tuberculosis screening before the last one expired and submitted it to the previous director. Staff #1 will request documentation of the tuberculosis screening from her doctor. If that cannot be obtained, staff #1 will submit to a new screening.
