Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 11:30 am - 1:45 pm. There were 36 children being supervised by 7 staff members. The children were observed participating in outdoor play, structured center play, eating lunch, and preparing for nap. Lunch served today is chicken patty on whole wheat bun, applesauce, peas, and milk. Allergy lists were posted in each classroom for children enrolled in the program with allergies. Allergy Care Plans were reviewed. Staff training was observed for completion of CPR, First Aid, and Virginia Pre-Service Training. Standard #: VENDSUB-000-006-B-11
Description: Based on observation of the food allergies and review of six children's records it was determined that not all children with a food allergy had documentation of a allergy care plan on file. Evidence: Child A, B, C, and D did not have an allergy care plan on file in regards to their diagnosed food allergy.

Plan of Correction: Administration will request an allergy care plan from the parents of Child A, B, C, and D.

Standard #: VENDSUB-000-012-E-1
Description: Based on observation of staff training it was determined that not all staff obtained CPR training within 30 days of employment. Evidence: Staff A with a start date of 10/09/2017 did not have documentation of CPR Training.

Plan of Correction: Staff A will enroll in a CPR Training course.

Standard #: VENDSUB-000-012-E-2
Description: Based on observation of staff training it was determined that not all staff obtained First Aid training within 30 days of employment. Evidence: Staff A with a start date of 10/09/2017 did not have documentation of CPR Training.

Plan of Correction: Staff A will enroll in a CPR Training course.
