Comments:
An announced inspection was conducted was conducted on December 27, 2017. The following areas were observed as a part of the inspection, physical plant, emergency procedures, emergency equipment and supplies, required documentation, program space, equipment and supplies and required posting's. Two staff records were also reviewed as a part of the inspection. The inspection began at 10:30 a.m. and ended at 1:25 p.m. The results of the inspection will be reviewed with the licensing administrator regarding a license. Violations:
Standard #: 22VAC40-185-320-B
Description: Based on observation and interviews, the restroom area provided for children did not include all the required items to include toilet paper, hand soap and disposable towels. Evidence: During the inspection on December 27, 2017, there were not disposable towels at the sink in the hallway.

Plan of Correction: Paper towels will placed at the sink today.

Standard #: 22VAC40-185-550-A
Description: Based on a review of documentation and interviews, the licensee did not ensure there was an emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation and shelter-in-place. The plan, which shall be developed in consultation with local or state authorities, addresses the most likely to occur emergency scenario or scenarios, including but not limited to natural disaster, chemical spills, intruder, and terrorism specific to the locality. Evidence: During the inspection on December 27, 2017, the center owner confirmed they have not discussed their emergency plan with the local authorities or had them review it.

Plan of Correction: The owner will contact local authorities this week.

Standard #: 22VAC40-185-550-B
Description: Based on a review of documentation and interviews, the licensee did not ensure the emergency preparedness plan contained all procedural components. Evidence: During the inspection on December 27, 2017, the emergency plan did not include the following; 1) Procedural components for sounding of alarms (e.g. intruder, shelter-in-place or chemical spill, fire) 2) Securing special healthcare supplies for evacuation and shelter-in-place 3) Staff training requirement, drill frequency, and plan review and update

Plan of Correction: The owner will update the plan today.

Standard #: 22VAC40-191-40-C-1-A
Description: Based on record review and applicant interviews, the applicant did not ensure the required background checks (Sworn statement or affirmation and search of central registry (CPS)) had been obtained at the time of initial application for the person who wants to operate a child day center. Evidence: During the inspection on December 27, 2017 the record for the applicant #1, did not include a completed CPS check and documentation indicates it was mailed on 12-5-2017. The sworn statement dated 12-27-2017 is not the most recent sworn statement required as it does not ask the question "Have you lived outside Virginia in the past five years?".

Plan of Correction: The owner will put the new sworn statement in the file today and will forward the CPS check once it has been received to the licensing office for review.

Standard #: 63.2-1701-B
Description: Based on observation and provider information, the child day center has not ensured that a person who constitutes, or who operates or maintains, a child welfare agency shall obtain the appropriate license from the Commissioner, which may be renewed. Evidence: During the inspection on December 27, 2017 it was confirmed through observation of children and interviews with the center owner the center is currently operating without a license. There were 9 children in care observed during today's inspection that began at 10:30 a.m. The center owner confirmed they purchased the business on October 1, 2017 and have been operating since October 2, 2017.

Plan of Correction: The owner is in the process of obtaining a Child Day Care license.
