Comments:
An unannounced monitoring inspection was conducted today 12/14/2017. The inspector arrived at 7:30 A.M. and departed the program at 10:00 A.M. There were ninety children present with twenty direct care staff members. During the inspection all eleven classrooms were observed, breakfast time was observed, infant care was observed, diapering was observed, hand washing was observed, nap time was observed, emergency supplies were viewed, eight medications along with authorizations were reviewed, policies and procedures were reviewed, and a sample of injury reports were reviewed.

Standard #: 22VAC40-185-160-A
Description: Based on review of 11 staff's records, the center failed to ensure that all records were complete with a TB screening with in 21 days of hire. Evidence: 1. The inspector found that staff member # 3 was hired on 8/23/2017 and there was no documentation of a TB screening.

Plan of Correction: The PD has spoke to staff # 3 and staff will go get a copy.

Standard #: 22VAC40-185-60-A
Description: Based on review of nine children's records, the center failed to ensure that all records were complete. Evidence: 1. The record of child # 8 did not have a documented start date.

Plan of Correction: The PD has corrected and will ensure to double check the files.

Standard #: 22VAC40-185-70-A
Description: Based on review of 11staff records, the center failed to ensure that all records were complete. Evidence: 1. The records of staff # 5 -8 did not a documented job title.

Plan of Correction: The PD has corrected and will ensure to double check the files.

Standard #: 22VAC40-185-350-C
Description: Based on observation and interviews with staff of the toddler classrooms, the center failed to ensure that all room were in the required staff to children ratio based on the youngest child in the grouping. Evidence: 1. Observed in the large motor room 13 children ages 16 months to years of age with two staff. The required staff to child ratio is one staff to five children. 2. Verified through interviews with staff the eleventh child arrived at 7:32 A.M. and the third staff person arrived at 7:37 A.M.

Plan of Correction: The PD wrote the staff person up that was late and explained the importance of being on time.

Standard #: 22VAC40-185-560-G
Description: Based on review of all foods brought from home, the center failed to ensure that all food met the standard. Evidence: 1. Two cans of opened formula in the little lambs room 2 were not dated and labeled with the owners name.

Plan of Correction: The PD has spoken with teacher and all formula has been corrected.
