Comments:
Licensing Inspector arrived at 11:40 AM and departed at 1:00 PM. At the time of the inspection, there were 32 children (ages infant through preschool) present with four staff. Children were observed sleeping. Infant care was also observed. The sample size was four staff and five children's records. Violations were noted in the areas of physical plant, staffing & supervision, and special care provisions & emergencies.

Standard #: 22VAC40-185-260-A
Description: Based on review of documents available at the facility, it was determined that the facility did not ensure that an annual fire inspection report from the appropriate fire official was available for review. Evidence: The most recent fire inspection available for review was dated 9/17/16 and was, therefore, over a year old.

Plan of Correction: Center management will ensure that an annual fire inspection is conducted and documentation is available.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation, it was determined that the facility did not ensure that a staff to child ratio of one staff for every four children was maintained when children under the age of 16 months were present. Evidence: During the inspection, there were 10 children, including infants, in care in the large infant room with two staff. Because infants were present there should have been a third staff member present.

Plan of Correction: Center management will review staff schedule to ensure that all ratios are maintained.

Standard #: 22VAC40-185-540-E
Description: Based on review of emergency supplies, it was determined that the facility did not have available a battery operated flashlight. Evidence: Although there was a flashlight available at the facility, there were no batteries available to operate it.

Plan of Correction: Center management will ensure that there are batteries available for the flashlight at all times.
