22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal inspection was conducted on 01/03/2018. There were 40 children, ages 5 years-13 years and 3 staff present during today's inspection. The inspector reviewed 5 children's records and 4 staff records. This program operates only when school is not in session and there was no medication being administered on the day of the inspection. The children were observed in the following activities: lunch, organized indoor large motor play, preparing to swim and during swimming. The inspector reviewed the following: the parent handbook, staff policies, emergency plans and emergency supplies. The inspector discussed the following with the staff: background check requirements, annual training hours, positioning of staff while changing for swim time, reporting injuries requiring outside medical treatment, and emergency drills. Standard #: 22VAC40-185-60-A Description: Based on record review, the facility failed to ensure that all children's records contain the information as required by the standards. Evidence: 1. The record for Child #1 did not contain the previous child day care attended by the child. 2. The record for Child #3 did not contain a complete address for both of the two required emergency contacts; one of the emergency contact addresses did not contain the city and state. 3. The record for Child #5 did not contain the name and phone number of the child's physician. Plan of Correction: The Child Care Coordinator will have the records corrected.
Standard #: 22VAC40-185-430-K Description: Based on observations, the facility failed to ensure that provisions were made for an individual place for each child's personal belongings. Evidence: The LI observed the children's backpacks, coats and lunch boxes scattered on the gymnasium floor as well as piled on top of one another on the gymnasium floor. Plan of Correction: The Child Care Coordinator had the children place their belongings inside of individual laundry baskets during the inspection. The baskets will be used at each Fun Camp Day.
Standard #: 22VAC40-185-550-D Description: Based on observation and document review, the facility failed to implement a monthly practice evacuation drill as required by the standards. Evidence: There was not an evacuation drill conducted in the month of December. The program was in session on 12/26/17, 12/27/17, 12/28/17 and 12/29/17. Plan of Correction: The Child Care Coordinator will have staff conduct and document drills each month.
Standard #: 22VAC40-185-560-F Description: Based on observation, the facility failed to ensure that when centers provide snacks a menu listing of foods to be served for meals or snacks during the current one-week period shall be dated, posted and kept on file for one week at the center. Evidence: The posted menu at the center was dated December 25-29, 2017. Plan of Correction: The Child Care Coordinator will put the new menu up.
Standard #: 22VAC40-185-560-M Description: Based on observations, the facility failed to ensure that staff shall sit with children during meal times. Evidence: The LI observed the 3 staff supervising the children standing and walking around during the designated lunch time; none of the 3 supervising staff members sat down at the tables with the children during the meal. Plan of Correction: A staff member will sit with the children at all snacks and lunches.
