Comments:
The Licensing Inspectors conducted an unannounced inspection on 11/9/17 at 10:15am in response to a self-report that was received by the Licensing office on 10/31/17 related to behavior guidance. Documentation was reviewed, observations of the classroom, and interviews were conducted. Based on the information gathered during the investigation for this self-reported incident, a violation for behavior guidance was cited and is listed on the violation notice attached to this summary.

Violations:
Standard #: 22VAC40-185-400-B
Description: Based on a review of documentation at the facility and interviews, it was determined that the facility did not ensure that behavioral guidance was constructive and administered in an age and stage appropriate way to redirect a child to appropriate behavior and resolve conflicts. Evidence: 1. On October 31, 2017 staff #1 flicked child #1 and child #2 in the head during nap time for talking and playing. 2. The parent of child #1 during an interview stated that their child said staff #1 came over and flicked child #1 in the head because she was playing and talking with another child during nap time. According to the parent. after being flicked in the head, child #1 started crying and staff #1 walked away. 3. The parent of child #2 during an interview stated that their child said staff #1 came over and flicked child #2 in the head because he was playing and not sleeping. According to the parent. after being flicked in the head, child #2 started crying and said it hurt.

Plan of Correction: The facility responded: Director has increased supervision for all classrooms - on going. Director has instituted more training opportunity for DAP behavior guidance for all staff - ongoing. Staff has been retrained on DAP
