Comments:
An unannounced renewal inspection was conducted on 01/11/2018. There were 23 children, ages 3 months-5 years, and 4 staff providing direct supervision present during today's inspection. The inspector reviewed 5 children's records and 5 staff records. The center does not administer medication. The children were observed in the following activities: centers, free choice activities, circle time, lunch and nap time. Infants were observed being fed on demand and sleeping on demand. The inspector reviewed the emergency preparedness plans, parent policies and staff policies. The inspector discussed the following with the provider: updates to the fingerprinting process, classroom arrangement, over-the-counter skin ointments, changing the medication policy, table sanitization, infant/toddler feeding tables, labeling items and nutrition. Violations:
Standard #: 22VAC40-185-440-B
Description: Based on observations and discussion with the staff, the facility failed to ensure that all cots are identified for use by a specific child. Evidence: Two cots in the toddler room and multiple cots in the pre-k room were observed not labeled in a way to identify use by a specific child. The cots have a plastic snapping device to label them but the device is easily removed by the children, the Program Director stated that the cots were labeled but that the children often remove the labels.

Plan of Correction: The Director will create a new labeling system for the cots.

Standard #: 22VAC40-185-570-A
Description: Based on observations, the facility failed to ensure that a high chair, infant carrier or feeding table is used for children under 12 months who are not held while being fed. Evidence: 1. The Licensing Inspector observed an 11 month old child lying on the carpet of the infant room drinking a bottle. 2. The Licensing Inspector observed a 9 month old child in an infant jumping chair in the infant room with a bottle.

Plan of Correction: The children will always be held or in the feeding table when eating.

Standard #: 22VAC40-191-60-B
Description: Based on record review and discussion with staff, the facility failed to ensure that all employees complete a sworn statement prior to employment. Evidence: The record for Staff #5 did not contain a completed sworn statement. The hire date for Staff #5 was documented as 01/10/2018. Staff #5 was observed working during the inspection.

Plan of Correction: The Program Director has Staff #5 complete the sworn statement during the inspection.
