- Technical Assistance:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, January 11, 2018 from approximately 8:40AM to 1:30PM. The following ratios were observed with a total of 49 children present during the inspection:
Infants - 10 children with 3 staff
Toddlers - 6 children with 2 staff
Twos (Discovery Preschool) - 9 children with 2 staff
Threes (Preschool) ? 14 children with 2 staff
Fours & Fives (Pre-K) - 10 children with 2 staff
School Age children were not present during the inspection.
During the inspection, the Infants were observed playing on the floor with toys, being held and fed, and being rocked to sleep. The Toddlers were observed participating in circle time where they sang songs. The Twos were observed playing dress up, building with blocks, playing with baby dolls, playing outside and participating in circle time. The Threes were observed playing in centers which included dramatic play, building with legos, looking at books, participating in circle time and playing outside. The Pre-K classroom was observed playing with animals, learning about the weather and reviewing Spanish words, and playing in the sensory table. Staff was observed interacting with the children and providing assistance when needed. Lunch was served during the inspection and consisted of chicken tacos, black beans, corn, fruit and milk. Later children began to transition into naptime. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children.
The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, daily attendance, daily sheets, parent agreements, and staff training information. Medication is administered and medication and medication authorizations were reviewed.

Standard #: 22VAC40-185-40-J Description: Based on documentation review, the facility did not ensure the injury prevention plan was reviewed and updated in writing at least on an annual basis. Evidence: During the inspection, the licensing inspector observed the injury prevention plan was documented as being reviewed and updated on 05/17/16. Plan of Correction: Staff will review the injury prevention and if updates are needed it will be added.
Standard #: 22VAC40-185-550-B Description: Based on review of required emergency preparedness plan, the required updates were not made.
During the inspection, the licensing inspector observed the listed primary designated contact person is a staff member that is no longer employed with the facility. The emergency preparedness plan listed previous staff members along with their phone numbers to be used in case of emergency.
Plan of Correction: We will review the emergency plan and make the necessary corrections.
Standard #: 22VAC40-80-120-E-5 Description: Based on observation and interview, the Notice of Intent (NOI) regarding intermediate sanctions was not posted in a prominent place at each public entrance of the facility. Evidence: During the inspection, the licensing inspector asked to view where the Notice of Intent (NOI) was posted in the facility. A member of management and the licensing inspector viewed the postings located at the front entrance of the building. The NOI was not posted. A member of management asked another member of management regarding the posting of the NOI and that staff member stated it was not posted. The NOI was mailed as a certified letter from the department on December 5, 2017. Plan of Correction: Corrected during the inspection.
