Comments:
A Subsidy Health and Safety inspection was conducted Tuesday, January 11, 2018 from approximately 8:40AM to 1:30PM to review supplemental health and safety requirements. The following ratios were observed with a total of 49 children present during the inspection:
Infants - 10 children with 3 staff
Toddlers - 6 children with 2 staff
Twos (Discovery Preschool) - 9 children with 2 staff
Threes (Preschool) ? 14 children with 2 staff
Fours & Fives (Pre-K) - 10 children with 2 staff
School Age children were not present during the inspection.
During the inspection, the Infants were observed playing on the floor with toys, being held and fed, and being rocked to sleep. The Toddlers were observed participating in circle time where they sang songs. The Twos were observed playing dress up, building with blocks, playing with baby dolls, playing outside and participating in circle time. The Threes were observed playing in centers which included dramatic play, building with legos, looking at books, participating in circle time and playing outside. The Pre-K classroom was observed playing with animals, learning about the weather and reviewing Spanish words, and playing in the sensory table. Staff was observed interacting with the children and providing assistance when needed. Lunch was served during the inspection and consisted of chicken tacos, black beans, corn, fruit and milk. Later children began to transition into naptime. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children. During the inspection, five children's records and five employees? records were reviewed.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11
Description: Based on record review of five children's records and interview, the facility did not ensure a child with diagnosed food allergies had a written allergy care plan that included instructions from the physician. Evidence: During the inspection, the licensing inspector observed the record for Child #2 did not contain a written allergy care plan that included all requirements. The record for Child #2 indicated the child had a food allergy. The record did not contain an allergy care plan that also included instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. A member of management also reviewed the records and the information could not be located during the inspection.

Plan of Correction: We have previously requested an allergy plan from the family. We will again request this information from the family and add to the record.
