Comments:
An unannounced monitoring inspection was conducted with the owner/director and assistant director beginning at 1:30 PM and ending at 4:25 PM. There were thirty-two children, ages 5 months through 4 years old, in the care during the inspection. The infants were observed sleeping in cribs and being held by care givers. The older children were observed sleeping on resting mats during nap time. Ten children?s records and five staff records were reviewed and in compliance. Two medications were reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent emergency escape drill was documented as January 4, 2018. The most recent shelter in place drill was documented as September 13, 2017. The most recent health inspection was documented as July 14, 2017. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not maintain all areas and equipment inside the center in a clean and safe condition. Evidence: There were areas of peeling latex paint on the walls of the toddler classroom which the children could easily pull the paint from the wall. There were small pieces of debris around and under the changing pad in the changing area.

Plan of Correction: The center is having the toddler and two year old classrooms painted on Saturday, January 20, 2018. The area underneath the changing pad was cleaned during the inspection. Staff will be retained on cleaning the changing area.

Standard #: 22VAC40-185-510-A
Description: Based on a review of documentation, the center failed to obtain written parental authorization for prescription medication. Evidence: Child A did not have parental authorization for two emergency medications (Epi-pen and inhaler).

Plan of Correction: The center will have the parent sign the authorization form.
