A Subsidy Health and Safety inspection was conducted on 01/10/2018 to review supplemental health and safety requirements. Today an unannounced monitoring inspection was conducted between the hours of 3:00pm and 6:00pm. I observed 23 children with 4 staff members. Ratios and supervision were in compliance. Children were observed eating snack, doing homework, using computers, and playing table games. Today's snack consisted of apples and yogurt. Staff were actively engaged with the children and assisting them with tasks as needed. The center was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Emergency preparedness supplies in order. Required documents posted. Record keeping was in compliance. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-004-F Description: Based on interview and observation, the center did not post, with parental approval, or keep in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: Three children currently attending the center have diagnosed food allergies. The center does not have a posted allergy list or a list accessible to staff containing information related to the children's allergies, sensitivities, or dietary restrictions. Plan of Correction: We will have a separate file and we will add an allergy list with their names, allergies, sensitivities, dietary restrictions. We will keep it accessible to staff.
Standard #: VENDSUB-000-006-B-11 Description: Based on record review, the center did not have a written allergy care plan for each child with a diagnosed food allergy, to include instruction from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child#1's record indicates that the child has a diagnosed food allergy. An Epipen has been provided to the center, but the center does not have a written allergy care plan on-site. Plan of Correction: We will ask parents to bring an allergy care plan from their doctors. We will attach the document to the allergy list for each child.
