An unannounced inspection was conducted from 3-430 PM. Staffing, ratios, program, and diapering was observed. The Inspector also reviewed records of children and staff. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on interviews and observation, it was determined the center has not maintained classrooms in a clean and safe condition as the classrooms have mice living under the heater's baseboards. Evidence: 1. The Inspector noted the paper that had questions about another treatment for mice. The Director was then overheard talking to a staff member that the Leasing office said they had treated once already. 2. Interviews with staff revealed the mice are still around and have been since the middle of December. Staff were asked the last time they saw the mice and two staff stated they saw them in the classrooms just the day before. 3. The Inspector noticed a few mice droppings in the two's class by the heater.

Standard #: 22VAC40-185-400-B
Description: Based on observation, a staff member did not demonstrate constructive behavior guidance or redirect children to appropriate behavior. Evidence: 1. The Director and the Inspector were present in the Two's class that consisted of 9 children and two staff. One staff member was observed handling diaper changes while staff #2 sat in a chair with the remaining 8 children. During this time, the Director had to redirect a child who was pulling a book cover off, retrieve a toy that another child dumped the small parts out and onto the floor, help a child pull shirt sleeves back in order to wash his hands, and take the Center's staff Ipad off the floor where children were playing. Staff #2 was told that the Ipad needed to be put up but did not stop from working on a child's hair. The Director had to then take the Ipad and place it on the upper shelf. Staff #2 also remained seated while a child was pushing a chair and using it to block the door. The Inspector then had to bend down and move the chair from the door.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the staff in the toddler room did not not ensure the diapering surface shall be used only for diapering or cleaning children. Evidence: 1. The Inspector observed a stuffed toy on top of the diapering surface and stated the toy cannot be kept on that surface. The staff then took the toy off and placed it on the shelf which is accessible to the children.

Plan of Correction: Not available online. Contact Inspector for more information.
