Unannounced inspections for a joint agency investigation were conducted on December 19, 2017, December 20, 2017, and January 10, 2018 in response to a self reported incident. Interviews with staff were conducted and records reviewed in response to an allegation that a two year old child was left unattended on the center's playground on December 11, 2017. The findings of the investigation confirmed the incident and are reflected on the Violation Notice . Please call with questions 703-479-4679

Description: Based on interviews and record review, it was determined that on December 11, 2017, the staff did not ensure the care, protection, and safety of a two year old child when they were unaware that the child was left unattended on the playground for approximately 10 minutes. Evidence: 1. Interviews with staff revealed that on December 11, 2017, at approximately 5:19 PM, Staff 1 and 2 went inside from the playground with 6 of the 7 children, ages 2 years, in their care. Each staff said they had to change diapers. Staff 1 took children in one room to change a child and staff 2 took children into another classroom to change child 1. Child 1's parent arrived at approximately 5:19 as staff staff 2 went into the classroom to change the child's diaper. After finishing the diaper change, staff 2 walked into the other class at approximately 530 and saw parent of Child 2 in the classroom with staff 1 and that the parent wanted to know where Child 2 was. Staff 1 did not have the child. Staff 2 ran to the playground and found Child 2 standing outside by the door and crying. 2. Staff 1 and 2 stated they did not use the face to name sheet when exiting the playground. 3. Attendance Record for December 11, 2017, was reviewed and the record shows when the parents sign in and sign out upon entering the building. Parent of Child 1 checked out at 5:19 PM which matched staff 2's statement that he saw the parent around 5:20. Child 2's parent signed out at 5:30 PM which is when staff 2 stated he was aware that the child was not inside with them. 4. An Incident report that was written by the Director states that the Incident occurred at 5:30 PM and the child was "left unattended on the two's playground".

Evidence: 1. Staff interviews revealed that Child 2 was left alone on the playground for approximately 10 minutes after staff 1 and 2 entered the center, from the playground, with 6 other children. Staff 1 and 2 were unaware that Child 2 had been left on the playground at approximately 5:19 PM until the parent arrived at center at approximately 5:30 PM.
