An unannounced renewal inspection was completed on 1/9/18. There were 91 children present with staff supervising. Ten children's records and nine newly hired staff records (six only reviewed completely - others reviewed for background checks only) were reviewed. Four daily medications and one emergency medication was observed and administration of the medication was also observed. The inspector arrived at the center at 8:00 am and departed at 12:45 pm. The children were observed during arrivals, during morning snack, during free times in gym, during outside time, during TV times, during story times, during lunch, and during preparation for nap. There was discussion with the director about the following: documentation of dates for staff leaving and returning to ensure TB screenings are completed within 21 days, information in the parent handbook related to the licensing office and the center's Board organization.
Standard #: 22VAC40-185-240-D-5 Description: Based on record review and interview, the center failed to ensure that there was at least one person on site at all times with daily health observation training and OSHA's blood borne pathogens regulation training. Evidence: 1. There was no documentation observed in the staff records of daily health observation training or blood borne pathogens training. 2. The director was asked about documentation of this training and stated that there was no documentation maintained for either of the trainings. 3. Daily health observation training and OSHA's blood borne pathogens training is required for at least one staff on site at all times to be updated at least every three years. This training includes: Components of daily health check for children; Inclusion and exclusion of the child from the class when the child is exhibiting physical symptoms that indicate possible illness; Descriptions of how diseases are spread and the procedures or methods for reducing the spread of disease; Information concerning the Virginia Department of Health Notification of Reportable Diseases pursuant to 12 VAC 5-90-80 and 12 VAC 5-90-90, also available from the local health department and the website of the Virginia Department of Health; and Staff occupational health and safety practices in accordance with Occupational Safety and Health Administration's (OSHA) blood borne Pathogens regulation. Plan of Correction: The director will provide and document this training for all staff.
Standard #: 22VAC40-185-560-G Description: Based on observation, the center failed to ensure that food from home was dated as required. Evidence: 1. The food table where all food from home is stored when brought in was observed at 8:15 am. The table had a date on it that the food was from 2/8/18 (yesterday). There were six lunch boxes observed on the table at 8:15 am. Plan of Correction: The staff who closes and ensures all food from home is put at the front desk for pick up will change the date for the next day of the food from home area when this is done.
