Comments:
An unannounced renewal inspection was conducted on 2/9/18 at 10:45 a. m. and concluded at 1:30 p. m. The inspector met with the staff and Child Day Center standards were reviewed. The following staff-to-child ratios and activities were observed. Infants: There were four infants with one staff. One infant was napping, one infant was being held and the other two were having free play. Toddlers: There were seven toddlers with two staff. The children were sitting at the table having their lunch. Two-Three Years: There were 16 children with two staff. The children were sitting on the floor and having circle time. Preschool (Three-Four Years) There were nine children with one staff. The children were preparing for lunch. Lunch consisted of Fish Sticks, Salad, Potato, Apple Sauce, and Milk. Ten children's records were reviewed. Ten staff records were reviewed. Classroom observations were conducted. Conversations were held with several staff and children. There were no medications on site today. The last monthly evacuation drill was conducted on 1/25/18. Four shelter-in-place drills conducted in 2017. Lock down done 7/21/17.

Standard #: 22VAC40-185-130-B
Description: Based on review of children records the center did not have all required information on file. Evidence: On 2/9/18, child #3 did not have updated immunizations on file.

Plan of Correction: Child had a conditional enrollment new to this center on 2/5/18. Parent advised updated immunizations must be received by 2/14/18.

Standard #: 22VAC40-185-140-A
Description: Based on review of children records the center did not have all required information on file. Evidence: On 2/9/18, child #1 and child #2 both enrolled over 30 days did not have a physical exam on file.

Plan of Correction: Exams have been requested by parent.

Standard #: 22VAC40-185-580-C
Description: Based on observation the center did not have all required information on one vehicle. Evidence: On 2/9/18, there were no emergency phone numbers available in one vehicle, this includes 911, police, emergency medical, and regional poison control numbers.

Plan of Correction: Numbers to be posted immediately.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on review of staff records the center did not have a background check on file for one staff. Evidence: On 2/9/18, staff #1 (start date 5/19/17) did not have a search of central registry on file. They are required within 30 days of employment.

Plan of Correction: Search was sent out and returned three times due to name issue. Will be resent on 2/9/18.
