Technical Assistance:
Discussed developing a spreadsheet to attach to your next renewal application to address the new staffing information required.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 28 children with 6 staff present along with the owner of the center. Observation of all three classrooms conducted. Classrooms were found well maintained, provided ample equipment, toys, art supplies and books, all within reach of the children in care. This allowed the children to freely access the supplies with independence. Artwork on display throughout the center. Seasonal artwork such as cotton ball snowmen and raccoon artwork to reiterate the lesson on the letter "R". Children were observed participating in a glitter project that simulated a tornado, coloring, making valentines and playing at the bean table. Lunch today was chicken alfredo, fruit and carrots. No medications on site at this time. All topical ointments reviewed and authorizations found complete. A total of 5 children's records, all staff records were reviewed. Standard #: 22VAC40-185-140-A
Description: Based on a review of 5 children's records, a physical exam was not found for one child record. Evidence: Child # 4 first day of enrollment was 9/25/2017. A physical exam was not obtained wiithin 30 days of enrollment.

Plan of Correction: We will request a physical exam from this child's parent. A speech and hearing exam was maintained along with immunizations prior to enrollment.
