Comments:
An unannounced renewal inspection was completed on 2/8/2018. There were 20 children were in care at the center, ages two years through five years old with four staff supervising the children. The inspector arrived at the program at 10:00 A.M. and departed the program at 1:30 P.M. Six staff's records and five children's records were reviewed. Informal interviews were held with all staff members and there were interactions with most of the children. A walk-through of the center and house was done. The children were observed in an inside activity, art time, music time, free play, outside time and lunch Discussion with staff about: completing the finger prints that was required with the code change 7/1/2017, storage of chemicals after diaper changes and ratio for two year olds.

Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records, the center failed to ensure that all immunization records were obtained prior to starting the program. Evidence: 1. The record of child # 1 started the program on 8/25/2017 and the immunization record was obtain on 9/28/2017.

Plan of Correction: In the future the immunization records will be obtain prior to starting the program.

Standard #: 22VAC40-185-150-B
Description: Based on review of five children's records, the center failed to ensure that all immunization records had a signature from a physician, his designee or an official of a local health department. Evidence: 1. The record of child # 4 started the program on 8/28/2017 and the immunization record did not have a signature or a date.

Plan of Correction: The administration will obtain a signed and dated copy and will in the future monitor.

Standard #: 22VAC40-185-160-C
Description: Based on review of six staff's records, the center failed to ensure that all TB screenings were updated every two years. Evidence: 1. The record for staff # 2 did not have a update TB screening. The most recent TB screening was completed on 3/3/2015.

Plan of Correction: The administration will have the staff person to complete a new TB screening ASAP.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center failed to ensure that all records were up-to-date. Evidence: 1. Child's # 1 record did not contain documentation that it was update and contained confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3.

Plan of Correction: The administration will ensure that all records are up-to-date.

Standard #: 22VAC40-185-240-C
Description: Based on review of six staff's records, the center failed to ensure that all staff had the required 16 hours of annually training hours . Evidence: 1. The record of staff # 1 had documented three training hours for the year of 2017.

Plan of Correction: The staff person is currently working on additional training hours and will ensure that this year all training hours will be obtained.

Standard #: 22VAC40-185-500-B
Description: Based on observation of the diapering process, the center failed to ensure that the table was cleaned and sanitized according to the standard. Evidence: 1. Observed the staff person using a sanitizing wipe on the changing pad. The standard requires soap and water and a santizing spray to be sprayed on the pad and to air dry.

Plan of Correction: The staff will make sure that the table is sanitized corretly after each change.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation of snack time and menu, the center failed to ensure that children three years of age or younger may not be offered foods that are considered to be potential choking hazards. Evidence: 1. Observed whole raw baby carrots being served for morning snack. The ages of children being cared for today are 2-5 years olds . 2. On menu for the rest of the month is mixed nuts which is considered a potential choking hazard.

Plan of Correction: The menu will be updated and will be given to parents.
