Comments:
An unannounced renewal inspection was conducted today from 10:00am-11:30am. At the time of inspection, 43 children (one year through five years) were directly supervised by 6 staff and ratios were in compliance. The sample size consisted of 5 children records and 5 staff records. Children were observed participating in group activities, reading books, creating Valentine's Day crafts, and eating snack (grapes and scrambled eggs). There was an abundant supply of materials for the children. Staff had current certification in medication administration, CPR and First Aid, as well as daily health observation training. Repeat violations were found and are identified in this report. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-270-A
Description: Based on observation, areas in the center were not maintained in a safe and operable condition. Evidence: 1. In the "Pre-Kindergarten 2" classroom, an area of chipping paint was found on the lower wall and a window was broken in a way that did not allow it to open properly. 2. In the "Beginners" classroom, the insulation surrounding the sink pipe was beginning to deteriorate and fall apart. 3. This standard was previously cited on July 6, 2016, February 15, 2017, and August 28, 2017.

Plan of Correction: All items being repaired on 2/15/18.

Standard #: 22VAC40-185-280-B
Description: Based on observation, potentially hazardous substances such as cleaning agents were not stored in a locked place. Evidence: In the "Beginners" classroom, two containers of disinfectant and a bottle of cleaning solution were found in an unlocked cabinet under the diaper changing table. This standard was previously cited on July 6, 2016.

Plan of Correction: The cleaners were locked up.

Standard #: 22VAC40-185-510-N
Description: Based on review of two medications, the center did not dispose of one medication or return the medication to the parent within 14 days of the expired medication authorization. Evidence: Child #1 had a medication available on site with an authorization form that expired on October 26, 2017.

Plan of Correction: Doctor is sending in a new one.
