The site director had specific questions about the lock down drill and plan. The location was observed and the plan was discussed. Two lock down drills have been practiced at the site. The site director also had questions about allergies/intolerances/insensitivities and requirements for each.
An unannounced monitoring inspection was completed on 2/20/18. There were 21 children present from four years through 11 years with three staff supervising. Five children's records and three staff records were reviewed. There were three emergency medications observed for one child in care. The inspector arrived at the school at 3:30 pm and departed at 6:00 pm. The children were observed during outside play time, during snack, during homework/reading time, during free time with table activities, during gym time, and during departures. There was discussion with the site director about the following: ensuring attendance is accurate at arrival of the children.
Standard #: 22VAC40-185-140-A Description: Based on record review, the center failed to ensure that each child had documentation of a physical examination completed under the direction of a physician within 30 days after attendance. Evidence: 1. Child #1 did not have documentation that a physical examination had been completed. The physical examination form was in the child's record but did not have any information other than the child's name, date of birth, and vital information. There were no other parts of the physical that had been filled out including the part which indicates if the child is well or has any conditions for school to be aware. Plan of Correction: The site director will talk with the school nurse to see if the physical that the school has is complete. If not, the site director will ask the parent to get a complete physical from the physician.
Standard #: 22VAC40-185-60-A Description: REPEAT VIOLATION Based on record review, the center failed to ensure that three of five children's records reviewed contained complete information that is required by the Standards. Evidence: 1. The record of child #1 was missing an address for one of two required emergency contacts for the child. 2. The record of child #2 was missing a complete address for one of two required emergency contacts. 3. The record of child #5 was missing the phone number for the child's physician. Plan of Correction: The site director will have the parents document the required information. The site director documented the physician's phone number during the inspection.
Standard #: 22VAC40-185-340-I Description: Based on observation and interview, the center failed to ensure that children were greeted upon arrival. Evidence: 1. An 11 year old child was observed coming into the cafeteria alone and unsupervised from the school bus that dropped the child off at this program. According to the site director, the school bus drops the child off at the driveway entrance to the school on the main highway in front of the school. The after school program entrance is approximately 50-75 yards from the highway where the child is dropped off. The child walked alone and unsupervised from the bus to the cafeteria. Two staff were passing out snack to the children and one staff person was talking with the licensing inspector when the child was observed to arrive. The site director was observed instructing a staff person to go out to the bus at 3:48 pm. The child arrived to the cafeteria at approximately 3:53 pm. Plan of Correction: The site director will talk with the transportation personnel about the bus coming through the school's traffic circle to drop the child off where a staff person will meet the bus. Until these arrangements are made, a staff person will meet the child where the bus drops the child off.
