22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted on 2/23/18 from 11:10am - 12:30pm. During the inspection there were 18 children ages one year old through five years old in care with six staff. Children were observed eating lunch and during rest period. Records were reviewed for five children and five staff. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-550-M Description: Based on a review of seven injury reports, it was determined that the facility did not ensure that there is a written record of children's serious and minor injuries in which entries are made the day of occurrence that includes all of the required items. Evidence: 1. The injury report for child #4 from 12/7/17, did not include the time at which the parent was notified about the injury. 2. Staff #1 (Program Director) reviewed the injury report for child #4 and confirmed it was missing the time at which the parent was notified about the injury. Plan of Correction: The facility responded: Teachers will be re-trained to include all of the required information. Center management will review all injury reports before they are given to parents to ensure they contain all of the required information.
