Technical Assistance:
Consultation was given on the new background requirements for finger printing, a revised Sworn Disclosure Statement for new staff hired after 7/1/2017 and requesting out of state Central Registry checks for staff who have lived outside the State of VA within the last five years. The model SDS form is online at VA DSS as well as a link to the Adam Walsh site to assist with out of state central registry forms. VDSS will be paying for initial child care fingerprint checks until 9/20/18. Each facility must establish a point of contact with DSS. All questions regarding facility codes should be directed to OBI at 804-726-7884. Eligible letters will be sent out by email and staff may not work with children (ratios or supervision) or be on site until a completed letter is received. Please read, review and respond to all correspondences regarding the fingerprint roll out.

Comments:
An unannounced complaint investigation was conducted jointly with the local Child Care Office. Based on the preponderance of evidence, the allegation for physical plan is founded and a violation was cited. In addition, a monitoring visit was conducted. Care of children, nutrition, emergency preparation, medication and records were reviewed. 59 children were in care with direct care staff and all rooms were in ratio. Observed infant care, circle time, transition to hand washing and preparation for lunch, reading with teachers and diapering. Areas of non-compliance are identified in the violation notice. Description: Based on center management statement, an area of the center has not been maintained in a safe and operable condition. Evidence: One of the lights in the Toddler room has been broken since 10/2017. Center Director has requested a work order to have the light repaired multiple times.

Plan of Correction: Maintenance was on-site 2/8/2018 and repaired the light by reinforcing the frame and ensuring it was no longer loose.

Description: Based on medication review, medication log was incomplete for one medication. Evidence: Child #2's medication was authorized for 5 days and the center medication log only documented the child receiving rhe medication for one day.

Description: Based on observation and staff statement, the posted menu did not reflect what was served for breakfast. Evidence: The menu dated 2/8 stated cornflakes and milk was to be served for breakfast and staff stated bagels with cream cheese and bananas was served to children. The menu on 2/7 called for french toast and cereal and apple sauce was served.
