An unannounced renewal inspection was conducted today from 4:00pm-5:00pm. There were 39 school aged children directly supervised by 4 staff. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and coloring. Children were beginning a long term project to build arcade games out of cardboard. Positive interactions were observed throughout the inspection. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-140-A Description: Based on record review, one child's file did not contain a physical. Evidence: Child 2's record did not contain documentation of a physical examination under the direction of a physician. Plan of Correction: We will have the parent provide us with the missing documentation.
Standard #: 22VAC40-185-60-A Description: Based o n record review, one of five children's records reviewed was incomplete. Evidence: Child 1's record did not contain one of of two designated people to call in an emergency if a parent cannot be reached. The second emergency contact's information was crossed out by the parent but not replaced with new information. Plan of Correction: We will have the parent update the information.
