Technical Assistance:
Form organization was discussed and action to take for allergies.

Comments:
Two inspectors conducted an unannounced renewal inspection from 9:45 a.m. until 11:15. There were 8 children in care with staff supervising at all times. The administrator was also present. Children were observed playing , eating snack, reading stories, and taking a nap. Three violations had been corrected and one violation was a repeat. Five children's records and five parent records were reviewed. The center does not administer medications. Please call your inspector at 276-698-9981 with any questions.

Description: Based on record review, the center did not have all the required information for each child enrolled. Evidence: 1. The inspector reviewed five children's records. Two out of five records did not have two emergency contacts. Child record #2 #3 did not have a second emergency contact listed.

Plan of Correction: The director will get the info. from the parents.
