An unannounced monitoring inspection was conducted on February 28, 2018. There were 117 children present at arrival to the facility in eight classrooms with 16 staff members. Additional staff members and volunteers were on site for administrative and support purposes. The children were observed having lunch which consisted of baked chicken legs, spinach, diced pears, whole wheat bread and milk. The children were also observed playing in centers, during music and movement, having storytime and napping. The outdoor playground equipment is not being utilized at this time. Ten children's records were reviewed. Two medications were reviewed.
Standard #: 22VAC40-185-90--A Description: Based on record review, in one of ten children's records reviewed, the licensee did not ensure that all required written signed agreements between the parent and the center shall be in each child's record by the first day of the child's attendance. Evidence: During the inspection conducted on February 28, 2018, the following written agreements were not in the record for Child #7: 1. An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds 2. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center 3. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. Plan of Correction: The case manager upon receiving their caseload will reviewed children's files for all documentation that needs to be kept in the files. All agreements for the child were faxed during the inspection from Central Files.
Standard #: 22VAC40-185-270-A Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection conducted on February 28, 2018, the following was observed: Room 103: The sink under the window constantly runs and the hot water trickles at the sink near the restroom. Room 106: The sink under the window constantly runs. Plan of Correction: We will resubmit work orders to Hampton City Schools to have that fixed.
