A Subsidy Health and Safety inspection was conducted on February 27, 2018 to review supplemental health and safety requirements. The inspector initiated the inspection at 10:00 am and concluded the inspection at 1:45 pm. The Owner/ Director was in attendance during the inspection. Upon the inspector's arrival children (Infants) in the main building were observed playing with soft blocks and interacting with staff. Diapering and feeding were also observed. In the second building the toddlers and preschool children were combined and listening to a story. Children also sang songs and talked about feelings, making happy and sad faces. Later children washed their hands and prepared for lunch. Today's lunch consisted of cheeseburger, bun, mixed vegetables, pineapple and milk. All areas of the center were inspected to include classrooms and bathrooms of both buildings and the playground area. The facility had the following required information posted: the license, last inspection notice, emergency numbers, evacuation map/procedures, daily schedules, allergy list and menu. The first aid kits for each building and for the van were checked and found complete. Non-medical emergency supplies were also inspected. The inspector interacted with staff and children in care.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Six staff records and seven children's records were reviewed.
Staff were on site with current CPR, First aid and daily health observation training and Virginia pre-service training (see violation notice).
Medications are not being administered. There are no staff with current MAT certification.
The most recent evacuation drill was documented on 01/16/2018. The two required shelter-in-place drills were completed on 04/13/2017 and 05/26/2017. A lock down drill will need to be completed (see violation notice).
Today's Ratios Observed:
Infants: 3 infants with 2 staff
Toddlers and Pre-school: 15 children with 3 staff

Fire Inspection: 11/28/2017 Health Inspection 01/11/2018

Standard #: VENDSUB-000-006-B-11
Description: Based on review of children's records, the facility did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction; Evidence: Child #8 with a diagnosed food allergy did not have an allergy care plan on record.

Plan of Correction: Per the Director:I will contact the parent today and have her supply by Monday.

Standard #: VENDSUB-000-012-B
Description: Based on review of staff records, the facility did not ensure staff completed Virginia Pre-service training within the first 90 days of employment. Evidence: 1. Staff #1, hired 08/10/2016, completed Virginia pre-service training on 04/18/2017 exceeding the 90 days from hire to obtain. 2. Staff #3, hired 02/01/2010, completed Virginia pre-service training on 12/10/2017 exceeding the 90 days from hire to obtain. 3. Staff #4, hired 02/21/2013 has not completed Virginia pre-service training exceeding the 90 days from hire to obtain. 4. Staff #5, hired 02/01/2016, completed Virginia pre-service training on 04/14/2017 exceeding the 90 days from hire to obtain.

Plan of Correction: Per the Director: Staff will complete by Friday, March 2, 2018.

Standard #: VENDSUB-000-012-E
Description: Based on review of staff records, the facility did not ensure staff obtained CPR and First aid taining within 30 days of the date of employment. Evidence: Staff #2, hired 09/11/2017 obtained CPR and first aid training on 10/19/2017 exceeding the 30 days from the date of hire to obtain.

Plan of Correction: Per the Director: I will ensure all new hires are certified within 30 days and renew current employees one month before expiration.

Standard #: VENDSUB-000-032-A
Description: Based on review of drill logs and interview, the facility did not conduct an annual lock down drill. Evidence: 1. There was no documentation of a lock down drill for the 2017 year. 2. The director stated that a lock down drill had not been conducted during the 2017 year.

Plan of Correction: Per the Director: I will perform one during the next five business days and annually there after.
