Comments:
A Subsidy Health and Safety inspection was conducted on Monday, March 5, 2018, to review supplemental health and safety requirements. Present today:
Infants: Sixteen children with four staff - Staff were observed sitting on the floor with the infants identifying colors and singing songs while other staff were feeding children and comforting others.
Toddlers: Fifteen children with three staff - Staff were assisting children in hand painting to make fish bowls for Dr. Seuss book, "One fish, Two Fish".
Two's Room 2- Thirteen children with two staff- Staff were assisting children in hand painting to make fish bowls for Dr. Seuss book, "One Fish, Two Fish".
Two's - 3 - Nine children with two staff - Children were singing songs and reciting their ABC's.
Two's - Room 4 - Eight children with one staff - Children were playing with cars, trucks and planes.
Preschool - 3's- Room 6- Eight children with one staff - Today the children discussed March, Monday and "Mm" words, built with hexagon blocks and practiced painting their names.
Preschool - 3's - Room 7- Eight children with one staff - Children were completing puzzles, pretending in dramatic play, building with legos and preparing for an art project.
Three's and four's - Room 8- Ten children with two staff- worked on letter recognition.
Pre-K- Room 9 - Thirteen children with two staff- The children were making hats for Dr. Seuss and learning the letter "Rr".

Lunch: Spaghetti with meatballs, mandarin oranges, broccoli, milk
Parent/Teacher Conferences were held in November 2017 and scheduled again for May 2018.
Ten children's records and ten staff records were inspected.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice.

Standard #: VENDSUB-000-012-E
Description: Based on review of staff records, the center did not ensure each staff record contained documentation of certification in first aid and CPR within 30 days of employment. Evidence: Out of ten staff records inspected, Staff #2, began working 3/2/17, and does not have documentation of first aid or CPR.

Plan of Correction: Per Center:
"Staff was unable to locate her first aid and CPR cards. She will re-certify at our next training in April 2018."
