The licensing inspector conducted an unannounced monitoring inspection Wednesday, March 7, 2018 from approximately 1:00PM to 6:00PM. The following ratios were observed with a total of 82 children present during the inspection: Infants - 7 children with 2 staff Toddlers - 8 children with 2 staff Toddlers/Twos - 7 children with 2 staff Twos/Threes - 14 children with 2 staff Preschool - 15 children with 2 staff Pre-K - 19 children with 2 staff School age - 12 children with 1 staff The children and staff were engaged in a variety of activities during the inspection. Children were observed sleeping at nap time, taking buggy rides, playing with toys, participating in center time, working puzzles, drawing, playing in the housekeeping area, playing outside and singing. All areas of the center including classrooms, hallways, playgrounds, and bathrooms were inspected. The facility was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, daily sheets, daily attendance, practiced emergency drill logs, parent agreements, staff training information and liability insurance. The center's first aid kit and emergency supplies were inspected. Medication is administered and medication and authorization forms were reviewed. Standard #: 22VAC40-185-60-A Description: Based on review of eight children's records, three records did not contain the required information. Evidence: During the inspection, the licensing inspector observed the last documented confirmation of up-to-date information for Child #4 was dated as 06/04/15. The record for Child #5 was dated 05/27/15 and the record for Child #7 was dated 10/04/16. Children's records are required to have at least an annual review to ensure the child's record is up to date. Plan of Correction: We will contact the parent and ask them to fill out an updated form.
