An unannounced monitoring inspection was conducted on Thursday, March 8, 2018. The following classroom staff to child ratios were observed:
Infant 1 - Eleven infants with three staff
Infant II - (16 months) Five children with one staff
Joey's Toddlers - (22 months) Four children with one staff
2's: Thirteen children with two staff
3's: Sixteen children with two staff
4's: Sixteen children with two staff
Children were waking up from nap when the licensing inspector arrived. Afterwards, observations were made including hand washing, snack time, diapering, children learning the letter "Mm" and making Rainbow Fish. Staff were observed interacting frequently with each child.
Lunch: Turkey and cheese sandwiches, applesauce, green beans and milk
PM Snack: Hummus and pita
A practice evacuation drill was conducted 2/28/18.
Standard #: 22VAC40-185-160-C Description: Based on review of staff records, the center did not ensure all employees resubmitted a TB (tuberculin) screening or testing at east every two years from the date of the first initial screening or testing.
Staff #3's TB results expired 2/8/18.
Plan of Correction: Per Center:
" System to ensure all staff have current and updated TB tests."
Standard #: 22VAC40-185-240-C Description: Based on review of staff records, the center did not ensure each employee completed 16 hours of annual training. Evidence: Staff #4 completed 11.5 hours of the 16 annual training hours required. Plan of Correction: Per Center:
"We will put a policy in place for the employees to complete training hours at 9 months to make sure they will reach the complete continuing ed. hours."
Standard #: 63.2-1720.1-A Description: Based on review of staff records, the center did not ensure new staff completed a background check in accordance with the Code of Virginia.
Staff #1 began working 2/26/18, and did not have the required background clearance prior to hire. Fingerprints must be completed and results stating the employee is eligible for hire must be present at the center prior to employment.
Plan of Correction: Per Center: "Employee will not work until fingerprinting is complete. We did not receive information from OBI but will contact tomorrow."
