An unannounced renewal inspection was conducted on March 5, 2018 from 7:15am-8:30am. At the time of inspection, 52 school age children were directly supervised by 2 staff members and ratios were not in compliance. A follow-up inspection was completed on March 6, 2018 at 7:40am to confirm that staff to child ratios were corrected. The sample size consisted of 5 children records and certifications for 3 staff members. Children were observed participating in group play, reading books, and playing board games. There was an adequate number of staff with current certification in medication administration, CPR and First Aid, as well as daily health observation training. Standard #: 22VAC40-185-350-E-5 Description: Based on observation and staff interview, the center did not maintain the required supervision ratio of one staff member for every 18 children. Evidence: On March 5, 2018, the Licensing Inspector observed 37 children with two staff members at 7:49 AM. At approximately 8:17 AM, there were 52 children with two staff members. A staff member confirmed the center supervisor was absent for the day and additional staff members were not available to maintain ratios. Plan of Correction: The partner center was unable to send coverage. Staff will communicate the need for coverage to the regional supervisor in order to ensure ratios are maintained.
Standard #: 22VAC40-185-420-E-3 Description: Based on a review of five children's records, the center did not request at least annually parent confirmation that the required information in the child's record is up to date. Evidence: The records for Child #1, #2, and #3 were last updated by the parents in September 2016. Plan of Correction: Parents will be asked to review and update their child's file at a minimum of once a year. The date the file is reviewed will be documented.
Standard #: 22VAC40-185-560-F Description: Based on review of the weekly menu, the menu was not dated as required. Evidence: The February menu was posted and available for parents on the date of inspection (March 5, 2018). Plan of Correction: The current monthly snack menu will be posted in a timely manner.
