Comments:
An unannounced monitoring inspection was conducted on March 9, 2018, from approximately 3:00pm - 4:45pm . There were 33 children present with two staff members. The children were observed participating in a variety of activities in the cafeteria and eating snack. The licensing inspector reviewed six children records.

Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on record review, the licensee did not ensure a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection conducted on March 9, 2018, child #1 did not have a written allergy care plan from a physician for there diagnosed food allergy.

Plan of Correction: The parent will receive the medication form to use as the action plan form.
