An unannounced Subsidy Health & Safety inspection was conducted today, March 13, 2018 to determine the center's compliance with subsidy requirements. The inspection was initiated at 3:17pm and concluded at approximately 5:45pm. All subsidy requirements were reviewed and compliance determined. Administrative records, all areas in the center used by children, and common areas were inspected.
A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: VENDSUB-000-006-B-11 Description: Based on the inspection of administrative records, the required information was not obtained. Evidence: In reviewing administrative records, it was discovered one child enrolled did not have an allergy plan from the child's physician. Subsidy, health, and safety requirements stipulates a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction should be obtained and available for review. Plan of Correction: Per director: I will obtain the information from the parent within the week. Once obtained, the allergy plan will be placed in the child's record.
