The new application was reviewed.

Comments:
The inspector conducted an unannounced renewal inspection at the center from 10:20 a.m. until 11:50 and at the main office to review six staff records from 1:05 pm-2:00. There were 30 children in care with staff supervising at all times. The cook and the family service worker were also present. Children were observed getting off the bus and entering the school, having a snack, participating in a dance activity, reading books, and having a bathroom break. Bus #3 was inspected. The playground was not inspected due to snow coverage. Violations are identified on the violation notice. Please call your inspector with any questions at 276-698-9981.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not keep all hazardous substances locked. Evidence: 1. The inspector observed a bottle of bleach on a corner shelf above the sink; the kitchen door is not kept locked. 2. The inspector observed a bottle of bleach on an upper shelf and a can of Lysol on the upper/high windowsill in the staff bathroom that is not kept locked. Substances were inaccessible to children but not locked.

Plan of Correction: The bleach in the kitchen was locked during the inspection. The site director will lock the hazardous substances in the staff bathroom.

Standard #: 22VAC40-185-280-G
Description: Baed on observation, the center did not clearly indicate contents of a substitute container. Evidence: 1. The inspector observed a spray bottle used for cleaning but it was not labeled. 2. The staff stated it was was soapy water and labeled it during the inspection.

Plan of Correction: Corrected during the inspection.
