Technical Assistance:
Discussed with the Principal outside vendors, trash cans in the preschool and kindergarten classrooms, damage to the playground fence that has been caused by a recent wind storm, and changes to background check requirements.

Description: Based on record review, the center did not obtain documentation of immunizations before a child can attend the center. Evidence: Child #1 first date of attendance was 2/6/2017 and their record did not contain documentation of immunizations.

Plan of Correction: Auditing child files and contacting families for missing information.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain documentation of a physical exam for children within one month of attendance. Evidence: Child #1 first date of attendance was 2/6/2017 and their record did not contain a physical exam. Child #2 first date of attendance was 9/6/2016 and their record did not contain a physical exam.

Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 record did not contain documentation of viewing proof of the child's identity and age. Child #2 record did not contain street addresses for both emergency contacts or documentation of viewing proof of the child's identity and age. Child #4 record did not contain street addresses for both emergency contacts. Child #5 record did not contain street addresses for both emergency contacts.

Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: There are various soaps and cleaning products located under the sink in the young infant classroom. There is no lock on the cabinet.

Plan of Correction: Relocated soap to locked cabinet

Standard #: 22VAC40-185-280-D
Description: Based on observation, cleaning materials were not kept separate from food. Evidence: There is a container of cookies stored in a bin along with cleaning materials in a cabinet in the Pre-K 2 classroom.

Plan of Correction: Reminder to staff. Food removed.

Standard #: 22VAC40-185-510-A
Description: Based on observation, medication was kept on site past the expiration date. Evidence: Child #8 emergency medication had expired 12/2017 and has not been returned to the parents.

Plan of Correction: Medication sent home with parent. New medication provided.

Standard #: 22VAC40-185-510-N
Description: Based on observation, expired medication was not returned a child's parents within 14 days of the authorization for medication had expired. Evidence: Child #2 medication authorization had expired 2/10/2018 and the medication had not been returned to the parents.

Plan of Correction: Medication sent home with parent. New medication provided.

Description: Based on observation, over-the-counter skin products were kept beyond their expiration date. Evidence: Child #6 had sunscreen that expired 12/2017 and a diaper cream that expired 2/2018 in the toddler classroom. Child #7 had a diaper cream that expired 12/2017 in the two year old classroom.

Plan of Correction: All staff checking current lotions, ointments, etc.

Standard #: 22VAC40-191-60-B
Description: Based on record review, one staff member had completed an unsatisfactory sworn disclosure statement. Evidence: Staff #1 completed a subsequent sworn disclosure statement on 10/19/2017. This staff member filled out an expired form that does not ask the employee if they have lived outside of the state of Virginia within the last five years. New sworn disclosure statements have been required as of 7/1/2017.

Plan of Correction: Had entire staff complete updated form.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, one staff record did not include central registry findings within 30 days of employment. Evidence: Staff #5 (date of hire: 8/16/2017) record does not contain documentation of a CPS check.

Plan of Correction: Have staff getting form notarized to send out.
