22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:11am-1:45pm. At the time of entrance, 222 children were in care with 48 staff members present. Children were observed during lunch time, diapering, story time and engaged in group activities. A selection of staff and children records, medications, emergency drill log and other documents were reviewed. The center is clean and organized. Standard #: 22VAC40-185-160-C Description: Based on review, the center did not obtain from staff at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors the results of a follow-up tuberculosis screening as stated in subsection B of this section. Evidence: 1. Staff A had TB test results dated 2/19/2016 and did not have an updated screening on file. 2. Staff B had TB test results dated 2/25/2016 and did not have an updated screening on file. Plan of Correction: Appointments have been made for both staff.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center inside were not maintained in a safe and operable condition. Evidence: 1. In Room 161, the children's play oven door had one hinge that has fallen off the door and is not hanging on properly. 2. In Room 159, the children's bathroom was missing floor tiles near the toilet and also on the side wall. 3. In Room 156, the trash can foot pedal was missing. The metal foot pedal bar was exposed. Plan of Correction: The play oven has been removed from the classroom. I have contacted our facilities minister and engineer and requested that is be fixed immediately. The trash can will be replaced today.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances such as cleaning materials, insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In room 159, there was a disinfectant spray and bleach water in the left high cabinet and two air wick sprays on the right high cabinet in an unlocked galley closet. The cleaning materials were out of reach of children, but were not kept locked. 2. In room 158, there was a bottle of bleach water on top of the towel dispenser in the classroom. 3. In room 237 and 238, there was a Lysol spray and two cleaning products on a high shelf in the galley area. The cleaning products were out of reach of children, but were not kept locked. 4. In room 239 and 240, there was a bottle of bleach water in an unlocked cabinet and a bottle of bleach water in the galley hanging from the cabinet. The cleaning products were out of reach of children, but were not kept locked. 4. In room 338, there was a Lysol spray, two cleaning products and bleach water in an unlocked cabinet. The cleaning products were out of reach of children, but were not kept locked. Plan of Correction: All cleaning products were moved to locked cabinets.
