On March 13, 2018, between the hours of approximately 10:30 a.m. and 2:30 p.m., an unannounced monitoring inspection was conducted at the facility. Twenty-seven (27) children were enrolled and twenty-three (23) children were in the care of six staff members. Six children's records and five staff members' records were reviewed and/or updated. No medications were on site/administered, on this date. Posted information, safety equipment (first aid kit, flashlight and radio) and available documents were observed/reviewed. The snack provided on this date included cheese, crackers, turkey, apple slices and water. Lunches were provided by the children's parents. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities observed on this date included: indoor play; snack; stroller walks and outdoor play; stories; restroom, diapering and handwashing procedures; lunch; and pick-up/departure of children. Staff/child interactions, behavior guidance and daily routines/transitions were observed. Staff members were observed to be positive, patient and supportive in their interactions with the children in their care. Children's independence was fostered/encouraged. Substantial compliance continued to be maintained with the licensing standards; one exception to compliance was noted on the accompanying violation notice. No repeat violations were cited during this inspection. Standard #: 22VAC40-185-130-B Description: Based on review and/or update of six children's records and discussion with staff on March 13, 2018, the program failed to obtain documentation of additional immunizations once every six months for one child under the age of two years. Evidence: 1. Review of child B's record, on this date, determined that documentation of additional immunizations was not obtained once every six months. 2. Immunizations for child B, under the age of two years, were last updated in August 2017. 3. Staff #2 confirmed the lack of a current immunization update for child B. Plan of Correction: The child's parent will be contacted and an updated immunization record will be obtained.
