Comments:
The licensing inspector conducted an unannounced monitoring inspection, at which lunch time, personal hygiene, rest time, and story time were observed. Children were observed; and staff and children files and other documentation were reviewed. Thirty-five (35) children, aged 2 months-5 years, were found to be in care with 7 staff, kitchen staff, and office staff. Previous violations were reviewed, and one (1) of the violations from the previous inspection had not been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 11:30am and concluded at 2:00pm.

Standard #: 22VAC40-185-140-A
Description: Based on review of the children records the facility failed to maintain compliance with this standard as required relating to physicals. Evidence: Child record #4, enrollment date 10/17/17, did not have a physical as required.

Plan of Correction: The guardian will be contacted today regarding the need for the physical. Once received it will be placed in the file for compliance.

Standard #: 22VAC40-185-60-A
Description: Based on review of the children records the facility failed to maintain compliance with this standard as required relating to allergies. Evidence: Child record #5, enrollment date 12/13/17, did not address any allergies the child may have.

Plan of Correction: The parent will be asked for this information upon pick up today. The information will be updated in the file for compliance.

Standard #: 22VAC40-185-330-B
Description: Based on review of the outdoor play areas, used by the children, the facility failed to maintain compliance with this standard as required relating to resilient surfacing. Non-compliance with this standard was previously cited on 09/26/2017 and therefore is marked as repetitive. Evidence: There was not adequate resilient surfacing under and around the play equipment, on all 3 play areas, as required by ASTM playground safety standards. The measurements were approximately 2-3 inches in depth.

Plan of Correction: More mulch will be obtained and placed in the areas where insufficient resilient surfacing is and fluffed or replenished for continued compliance.
