An unannounced monitoring inspection was conducted on 3/19/18 from 9:36am - 12:00pm. When the Licensing Inspector arrived there were 50 children ages two years old to five years old in care with 10 staff. Children were observed participating in various activities in the classrooms, playing outside, and eating snack. Records were reviewed for five children and five staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-330-B Description: Based on observation, it was determined that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Evidence: 1. There was exposed pieces of broken concrete under both swings on the playground, and the resilient surface (mulch), was less than the required six inches. 2. There were two plastic portables slides that did not have a six feet fall zone on all sides. One slide was approximately one foot from the barrier used to hold in the resilient surface, and the other slide was approximately two foot from the same barrier. 3. Staff #6 (Program Director) viewed the resilient surfacing under the swings and the location of the portable slides, and confirmed that it was not compliant with the requirements in the standards. Plan of Correction: The facility responded: A work order will be placed to have any concrete removed, and more mulch added to the playground. the slides will be moved today to ensure there is six foot fall zone on all sides.
