An unannounced Subsidy Health and Safety Inspection was conducted on March 21, 2018 from approximately 9:50 a.m. until 1:10 p.m Upon entrance there were 21 children and 3 staff present with children. There were additional staff on site for administrative and program purposes. The record review sample consisted of 5 children's records and 5 staff records. Children were observed during their chapel program. Please contact the licensing inspector with any questions (757) 404-0568.
Standard #: VENDSUB-000-004-F Description: Based on observation and interviews, the vendor did not ensure that a list of children with food allergies is posted with parental approval, or kept in a way that is immediately accessible to staff, a current list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: During the inspection on March 21, 2018, posted in 2 of 2 of the classrooms was an "allergy list" with 7 children's names (child #5,#6, #7, #8 ,#9, #10 and #11) on it. The center had not obtained parental permission to post the list with the children's names as having a food allergy, sensitivity or intolerance. Plan of Correction: The center director will obtain written permission or cover the list posted.
Standard #: VENDSUB-000-006-B-11 Description: Based on observation and interviews, the vendor did not ensure there was a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on March 21, 2018, 2 out of the 4 records reviewed included documentation that child #3 and child #4 have diagnosed food allergies and their records do not include documentation to include physician's information on how to address an allergic reaction.. The documentation indicates child #3 is allergic to milk and child #4 is allergic to peanuts and tree nuts. Plan of Correction: The center director will talk with parents about the allergies and get updated forms .
Standard #: VENDSUB-000-031-B-1 Description: Based on observation and interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an emergency to include evacuation, shelter-in-place and lockdown emergencies. Evidence: During the inspection on March 21, 2018, there were not any reunification plans documented in the emergency preparedness plan for evacuation, shelter-in-place and lockdown emergencies. . Plan of Correction: The center director will incorporate it in the new updated plan and include phone numbers where they can be reached.
